This collection includes: photographs, publicity publications, magazines, papers, notebooks, VHS tapes, DVDs, cassette tapes, and apparel.

Sylvia Lewis' Personal Records include personal correspondence spanning her career, collected newspaper and magazine clippings (called "puff" pieces) concerning Sylvia Lewis' personal life and career, collected tabloid magazines featuring Sylvia in its coverage, and personal headshots taken of Sylvia during her career.

Specific records relate to Sylvia's involvement with "Tucker Smith Day", a benefit held in honor of her friend Tucker Smith, "Luigi's 65th Birthday", a surprise celebration held in honor of her colleague and friend, and "Huis Ten Bosch", which was the name of a theme park in Japan where Sylvia choreographed the opening ceremonies. There are records tracking Sylvia's involvement with Ralph Faulkner and Falcon Studios, and records that follow Sylvia's involvement with several tribute and remembrance pieces during the 1990s and 2000s.
