This collection includes: photographs, publicity publications, magazines, papers, notebooks, VHS tapes, DVDs, cassette tapes, and apparel.

Sylvia Lewis' Personal Records include personal correspondence spanning her career, collected newspaper and magazine clippings (called "puff" pieces) concerning Sylvia Lewis' personal life and career, collected tabloid magazines featuring Sylvia in its coverage, and personal headshots taken of Sylvia during her career.

Specific records relate to Sylvia's involvement with "Tucker Smith Day", a benefit held in honor of her friend Tucker Smith, "Luigi's 65th Birthday", a surprise celebration held in honor of her colleague and friend, and "Huis Ten Bosch", which was the name of a theme park in Japan where Sylvia choreographed the opening ceremonies. There are records tracking Sylvia's involvement with Ralph Faulkner and Falcon Studios, and records that follow Sylvia's involvement with several tribute and remembrance pieces during the 1990s and 2000s. There are also specific papers concerning guitar lessons that Sylvia took during the 1970s, and programs relating to a 1970 art showcase of one Lazar Obican.

Audio and visual records are included, which record Luigi's 65th Birthday Celebration and Sylvia's time in Huis Ten Bosch. Apparel items include a red gown, worn by Sylvia at the Emmy Awards in 1958, a "high-end couture hat", a jacket from the opening of Huis Ten Bosch, a yellow "wedding" dress, and a series of rehearsal and performance shoes (tap, ballet, and pointe) that Sylvia has kept over her years in performance. The shoes are included in the "Personal Records" series because Sylvia provided personal insight into how she kept and wound her shoes. Individual apparel items have been described individually.
